Anything goes as long as anything overly graphic/violent/sexual is marked as such (basically, use your judgment). I know some people like to list wordcount, etc.

I may make a 'suggested header' if there seems to be a problem with not having a standard format, but for now, if you have one you prefer to use, definitely go with that.
